Your booking is secure when I have full payment and sent an email confirmation of this.


Cancellations

 

The physiotherapy fee for face to face appointments is payable at the end of the session. The physiotherapy fee for online appointments is payable at the time of booking. I have a 24 hours cancellation policy; if you are unable to attend the appointment, please cancel a minimum of 24 hours before it is due to begin. If you cancel within 24 hours, then you will not be entitled to a refund.

The fee for the online courses is payable at the time of booking and is non-refundable.


Technical requirements for online courses

 

We ask that you have a facebook account and have a Zoom account set up and tested before the course begins.


Code of conduct

 

Pro Pelvic Floor workshops and online courses are supportive and friendly. I ask that you are thoughtful and respectful of the group, and do not share other people’s information outside of the group.

Pro Pelvic Floor reserves the right to exclude from a workshop or online course any person who behaves in a way that is abusive, disruptive or discriminatory. No refund will be given.

 

Privacy and Data Use

How we use your data

 

Before an initial physiotherapy appointment, I ask for your name and email. This is to set up a new physiotherapy record and contact you about your upcoming appointment or online course.

Online classes are run jointly by Pro Pelvic Floor and Fitz and Physio. Initially we ask you to fill out an application form. This is to check for any major health concerns and suitability to join our programme. 

The Newsletter provides general information about pelvic health and information about the new things happening at Pro Pelvic Floor. To  sign up, I request your name and email address. You can request to stop receiving this newsletter at any time.

You can remove your consent for me to contact you at any time. Your data is collected and held in accordance with UK data protection regulations. 

I am a HCPC and CSP Chartered Physiotherapist and I have a legal obligation to keep your records for 8 years since the last date of treatment or date you finished your online course. This does not apply to the data I hold for the newsletter which is only held for as long as you wish to receive the newsletter. You can ask for a copy of the data I hold about you and once we have verified your identity, I will respond within 28 days.

You can request a copy of the data held about you at any time, free of charge. Once I have verified your identity this will be provided within 28 days.
